Private Car Service Agreement
Any and all corrections to this reservation are needed within 24 hours and or before time of service. Client is responsible to be ready at designated location prior to scheduled pickup time. A no show or missed car service may be applied after the grace period of 5 minutes is surpassed and will be billed in full. Wait times may be available if needed; and will be charged $1 per minute. To cancel service a cancellation request must be submitted and confirmed via email 72 prior to service, a $20 cancellation fee will be applied to cancellations. After hour fee my apply from 11:00 P.M. – 6:00 A.M. Cancellation requests received within 72 hours charged at full price. Rescheduled or changed reservations subject to a $20 charge. Reservation is for point to point only. If the transfer exceeds the reserved point-to-point geographic area additional charges may apply. Affinity Transportation is not liable in the event of mechanical breakdown while on charter and will only be responsible for making up lost time at a mutually agreed date. Client assumes full financial liability for any damage to the vehicle caused during the duration of rental by them or any members of their party. Out of service cleaning fee including but not limited to vomiting = $500 minimum. Smoking is not permitted in any of our vehicles. Affinity Transportation is not financially liable for missed engagements. Affinity Transportation is not responsible for delays or run cancellations due to driving conditions. (i.e. heavy snow/rain, accidents, traffic, etc.). Affinity Transportation is not responsible for items left in the vehicle. Any remaining balances must be paid to the driver before the commencement of service. In rare circumstances Affinity Transportation may need to cancel a reservation for reasons not listed, if so client will be issued a full refund of any money paid to Affinity Transportation. Payment to Affinity Transportation by the client denotes an agreement to the above Reservation Policy and the client agrees to be bound by these terms and conditions in lieu of a signed copy.
Out of State / Area Service Agreement

A $100 nonrefundable deposit is required at time of reservation. To cancel service a cancellation request must be submitted and confirmed via email 7 days prior to service. Cancellation requests received within 7 days of service will be charged at full price. Rescheduled or changed reservations subject to a $75 charge and cannot be changed within 7 days of service. Limousine Service Agreement
If any corrections are needed please contact Affinity Transportation within 24 hours and or before time of reservation. Client is responsible to be ready at designated location prior to scheduled pickup time. 1 . A full deposit is due at time of reservation. (all deposits are NON refundable ) 2 . Affinity Transportation is not liable in the event of mechanical breakdown while on charter and will only be responsible for making up lost time at a mutually agreed date. 3. Alcohol consumption under the age of 21 and or drug use is prohibited and the client will pay for any related fines. 4. It is Illegal to stand through the emergency exit. 5. Client assumes full financial liability for any damage to the vehicle caused during the duration of rental by them or any members of their party. Smoking is not permitted in any of our vehicles. (see #6) 6. Damage fees: Out of Service cleaning fee including but not limited to vomiting = $500 minimum • Smoking inside the vehicle = $200 minimum. • Carpet burn = $200 minimum. • Miscellaneous damage (windows, handles, CD/DVD players, etc.) = $250 minimum. 7. Overtime pay billed in hourly increments) is applied as soon as the prearranged time stated on the trip sheet is surpassed. Affinity Transportation is not financially liable for missed engagements. 8. Affinity Transportation is not responsible for delays or run cancellations due to driving conditions. (i.e. heavy snow/rain, accidents, traffic, etc.). 9. Affinity Transportation is not responsible for items left in the vehicle. 10. Any remaining balances must be paid to the driver before the commencement of service. 11. Vehicles cannot be loaded beyond seating capacity. 12. The driver has the right to terminate limousine service without refund if there is blatant indiscretion on the part of the client(s) 13. All no show or missed car service by the client will be billed at full price. In rare circumstances Affinity Transportation may need to cancel a reservation for reasons not listed, if so client will be issued a full refund of any money paid to Affinity Transportation. 14. By making a deposit to Affinity Transportation , you the client agree to the above Reservation Agreement and you the client agree to be bound by these terms and conditions. The receipt of any deposit by Affinity Transportation is considered acceptance of these terms in lieu of a signed copy.
